What is the Unio Authorization Form?

The Unio Authorization form is a legal document completed by the resident/patient or their legal representative. This form gives an individual permission to access the resident/patient's health care information in the Unio Care app.

Why do we need the Unio Authorization Form completed?

The Health Insurance Portability and Accountability Act (HIPAA) requires authorization/consent to release another individual’s protected health information (PHI). Due to this requirement, this form must be completed before a Unio Care user can see resident/patient health information in the Unio Care app.

Who can complete the Unio Authorization Form?

The resident/patient must complete this form at their care facility. If the resident/patient cannot complete this form, their legal representative may complete it on their behalf. Please get in touch with your facility about the completion of this form.
