How to Request to Connect to My Loved One

Learn how to request to connect to your loved one's care profile, and what information is required.

II. Request to Connect to My Loved One

If you have not yet received a personal invite code to connect to your loved one’s care profile, you can request to connect to your loved one's care profile within the Unio Care app.
  1. Sign up for Unio Care. When you register, enter your personal information as you have given it to your loved one's care facility.
  2. Verify your email address.
  3. Tap on the “Request to connect to my loved one” link.
  4. Enter your loved one’s information. You must enter their:
    • First and last name
    • Date of birth
    • Facility they are a residing in
  5. Enter your information. You must enter your:
    • Relationship to your loved one
    • Phone number the facility has for you
  6. Tap “Submit.”
Once you submit a request to connect, we will verify the information in the request and prompt your loved one's facility to provide your loved one or their responsible party with the Unio Care authorization form to be completed.

III. Check the Status of My Request

You will receive email updates if there is any additional information needed by our support team to complete your request, but you can also check the status of your request from within the Unio Care app. To do so, simply:

  1. Open the Unio Care app.
  2. Tap on “Check my request status.”

If you have any questions regarding the current status of your request to connect to your loved one's care profile, please reply to the email you received from our support team, or contact families@uniocare.com.


IV. Unio Authorization Form

The Unio authorization form is a federally required document that a resident or their responsible party must sign to provide Unio Care with the consent to share their protected health information with an authorized user. 

What is the Unio authorization form?

The Unio Care authorization form is a legally required authorization or consent for the release of health information that is protected by federal law. It can be found at your loved one's care facility, or sent to responsible parties via DocuSign.

The authorization form needs to be filled out with two important pieces of information: the resident's name and date of birth, as well as the name of the requested authorized user, and their relation to the resident. It must also be signed by either the resident or their legal representative, with all consent boxes checked to be considered valid.

Residents or their legal representatives may request a signed copy of any submitted Unio authorization forms by contacting families@uniocare.com.

If you are the responsible party or legal representative for your loved one, you may contact the Unio Support Team at families@uniocare.com, or your loved one's care provider for blank copy of the Unio Care authorization form.
